Health and Physical Well-being survey

The following consultation survey consists of questions that collect readiness and receptiveness of parents and whanau on Health and Physical Education Curriculum implementation at Wairakei Primary School.

Please note we are aware not all Wairakei Primary School community members will be comfortable with questions on health, physical well-being and sexual education. We want to advise this survey contains questions that may be considered sensitive. If you prefer an alternative culturally-responsive approach please contact Paula Farquhar directly.

1. Do you believe Health and Physical Education should be taught at Wairakei Primary School to help our students maintain or improve their well-being?
2. I believe there is the need for Wairakei Primary School Year 6 students to learn about Changes at Puberty.
3. I believe it is important for students to learn about mental health
4. I believe it is important for students to learn about well-being
5. I believe it is important for students to learn about body care and physical safety
